New Patient Registration

We are always happy to accept new patients and can register people who live within our practice area (which covers the town centre and the villages of Henley, Claydon, Bramford, Sproughton and Burstall).


Please ask reception for further advice or clarification. Please make sure that you have proof of identity (e.g. driving licence or passport) and local residence (from within the last 3 months) if you intend to register as a patient.


Patients undergoing treatment or on medication will need to book an appointment with the Doctor before the next repeat prescription is due. There will be three forms that we require you to complete on registering, all available to download and print below.


If you are on repeat medication, please bring your repeat slip from your previous GP, with your registration forms.

Please ensure that you complete all 3 forms below to enable us to complete the registration process;


1. Family Doctor Service Registration (GMS1)

2. New Patient Questionnaire (Adult) or New Patient Questionnaire (Under 16)

3. Who Can See My Health Record
